3032 - Fringe Benefits to Employees
- Individuals employed by Educational Service Unit 4 on a Full Time Equivalency (F.T.E.) of not less than 50%, shall receive compensation equivalent to Family PPO Blue Cross/Blue Shield Health Insurance, $950.00 Deductible* with PPO 80% A & B with 50% C Dental coverage, prorated according to their actual F.T.E., i.e. - employee working an F.T.E. of 75% will be eligible for 75% of the compensation equivalent. Employees who work less than full time will be responsible for providing the remaining amount if they desire the health insurance.
At the employee's discretion, this compensation may be utilized for health insurance or 403(b) contribution. This compensation will not be awarded to the employee in the form of cash. The amount contributed to the 403(b) will be calculated according to gross salary and years of service with ESU 4. Individuals with 1-4 years of service will receive 20% of their gross salary; 5-9 years of service will receive 25% of their gross salary and individuals employed for years thereafter will receive 30% of their gross salary. This compensation will not exceed $1,100.00. The employee shall furnish a written and signed statement concerning the disposition of this compensation. - All individuals employed by Educational Service Unit 4 shall receive long term disability coverage based on the individual's current gross salary.
- All individuals employed by Educational Service Unit 4 shall receive group life insurance with a maximum coverage of $30,000.00.
*Effective September 1, 2009, the ESU 4 Board agreed to provide health insurance at a $950.00 deductible.
